Missing CE Certificates or Post-Conference Assessments for the ATAF Clinical Symposium
Any attendee who needs access to the post-conference assessments (EBP or Category A) for an ATAF Clinical Symposium should email ATAF Treasurer (treasurer@ataf.org) and Vice President (vicepresident@ataf.org) with his/her request. Please include: full name, email address, conference year, session assessment(s) name(s), and NATA membership number. ATAF will assess a $25 fee for re-issuing of post-conference assessments/CE statement of credit documents for each Symposium (all category/session surveys included). To avoid reporting delays for the Board of Certification and/or Florida Department of Health, participants should allow a minimum of 30-days for ATAF to process the request. Requests made after December 1 will not be processed until January.
